When Do You Need This Form?
When applying for a new EU Blue Card or renewing an existing one. You must have a completed university education (or 5+ years of professional experience in certain fields) and a job offer paying at least 1.5x the average Czech gross salary.
Required Attachments
- Valid passport
- 2 passport photos (3.5×4.5 cm)
- Employment contract for at least 1 year
- Proof of higher professional qualifications (university diploma)
- Proof of accommodation
- Clean criminal record (not older than 180 days)
