Application for Employee Card (Issuance or Renewal)

Application for EU Blue Card (Issuance or Renewal)

Application for the EU Blue Card — a residence and work permit for highly qualified third-country nationals: a university degree, at least 3 years of higher vocational education, or comparable professional experience, and a contract paying at least 1.5 times the average gross annual wage the Ministry of Labour announces for Blue Cards.
EU Citizen: Application for Registration Certificate

Registration form for EU/EEA/Swiss citizens planning to stay in the Czech Republic for more than 3 months. Also applies to qualifying UK citizens under the Withdrawal Agreement.
Long-term Visa Application (over 90 days)

Application for a long-term visa (D-type visa) allowing stay in the Czech Republic for more than 90 days. This is the first step for most third-country nationals arriving for study, work, or family reasons.
Proof of Accommodation (by Individual Landlord)

The official form for individual landlords to confirm they are providing accommodation to a foreigner. Required for virtually all visa and residence applications.
Employee Card: Notification of Employment Termination

EU Family Member: Application for Temporary Residence Permit

Permanent Residence for EU Citizens & Family Members

Employer Confirmation: Foreigner Has Commenced Employment

Employment Income Certificate

Power of Attorney
Plná moc
Form granting another person the authority to represent you at the Department of Asylum and Migration Policy (OAMP). Useful when you cannot attend appointments personally.
Blue Card: Notification of Employment Termination

Application for EC Long-term Resident Status

Unified Confirmation of Acceptance for Study

Official form that Czech universities fill out to confirm a foreign student has been accepted into a study program. Required for student visa/residence applications.
